Infinite-loop/duplicate-output detection and hard-interrupt plugin for DeepSeek Harness (dsh 0.2.x): text duplicate detection + intent-level duplicate detection for tool calls; upon detection, performs a hard cancel interrupt and uses steer to guide a retry
