Hot Reloader

DSH plugin hot reloader: changes to plugin source take effect immediately without restarting DeepSeek Harness. Covers third-party plugin cases that the host's built-in dsh-hmr cannot handle because it excludes node_modules. Hot-reload DSH plugin source without restarting the harness.
